Perhaps I misunderstand your question, but the note icon already shows 
whether or not a note exists.  For a selected row, if there is no note, 
then the note icon has a plus sign: "click me to add a note".  If there 
is a note, then the note icon has a disclosure triangle pointing left 
or down depending on whether the note is displayed.

The plus sign or disclosure triangles only appear if the row is 
selected.  Otherwise, the presence of a plain note icon indicates the 
presence of a note.
