columns and numbering in parts of rows only?

I doubt it, but it would sure be handy -- I often have multiple columns 
where the data contained in columns 2 through N applies only to items 
at a certain level in the hierarchy. I would like it (and have 
requested it) if I could make certain levels of the hierarchy behave as 
in line notes do now -- spanning the width of the document.

That said, the current implementation of in-line, spanning notes is 
pretty nice until we get that.
