Just a wild guess, but could it be that there are applications which 
change
these settings? As viable candidate, GraphicConverter comes to mind 
because

a) it is pre-installed in many recent Macs
b) it does ask to change the default application for a couple of 
extensions.

So some application might be aware of the QT settings, and might have 
changed
that. I just checked, and most are checked, but not "PDF Image"...
